To enter, please download this form, fill it out and mail it in along with your application package, which must include:

1. The final version of an essay or a report written for school, as well as the first unedited version of the same essay or report.
2. The final version of a creative writing sample such as a short story or a poem, as well as the first unedited version of the same short story or poem.
3. A short writing sample (under 250 words) completed without assistance.
4. A personal reflection (under 500 words) on any topic of your choice.
5. A signed and completed Eligibility and Original Work Statement form.
6. Please label your writing pieces to correspond with the numbers above ( #1, #2, #3, and #4) and submit along with your Entry Form and Eligibility and Original Work Statement.
