Note: Speech and Language Services are only for currently enrolled NTID students or students who will be enrolled by the time they receive services.

Using this request form, you can do the following:

• You can sign up for Individual Speech Therapy or just request a meeting with a Speech and Language Pathologist to find out what is best for you.

What will happen when you use this form?

• You will be contacted and told that your request has been received.
• You will be contacted by a Speech and Language Pathologist who will meet with you.
• You will be asked to send, or bring, a copy of your complete class schedule for the first meeting.
